Pouring over old concrete causes an effect known as mirroring, which means all the defects in the existing concrete will mirror through to the new surface. This can be stopped by use of a bond break material over the old concrete, then pouring your new concrete pad a minimum of 4" thick, provided you have enough height to do so. I would also recomend using fiber mesh add mixture as added by your local concrete company.If you should decide to go with a repair you can view our website for info and links to manufactures of concrete repair products.
Always start w/ your local city hall. Permits are required for most all types of construction and remodeling projects. As a general rule all structual and mechanical systems require permits, plumbing, electrical, heating, framing, ect. However, non structural and non mechanical systems such as paint, trim, window replacement, carpet ect. USUALLY do not.

You need to speak w/ an engineer to decide on a beam size and type.Some good lumber yards offer this service free if your purchasing the beam from them. I strongly recommend finding a professional local contractor.

The possibilities as to cause are too numerous . You need to start with a local engineer for a report, as well as a remedy. In all probability you will need an engineer's report or drawing when applying for your local permit.

There are too many variables involved to just give a blanket answer. You should check w/ your local supply yard for an expert in your area to help with this decision. If removing the old you could even cause damage to the smoke chamber if done incorectly.

Have you considerded a masonry heater ? In an 800 sqft home this would probably work well. You can read about them on our web site @ brickovens/masonryheaters

A Fire Box is only part of a real fireplace . If you don't have one then you probably need a complete fireplace installation. You should research local qualified masons to do the installation
